How do I obtain an application form?


Is there a separate application form for out-of-state trained applicants?

No. The application forms [PDF] and the renewal forms [PDF] can be used for requesting all types of certification.


Which box do I need to check for the type of credential I am seeking?

Do your best to select the type and/or name of the credential, certificate, or permit using the check box(es) on page 1 of the application form [PDF]. If applicable, include any subjects, emphases, and/or supplementary or subject matter authorizations. If you do not find a box appropriate to the credential type you are seeking, simply write your request in any available blank space on page 1 of the application [PDF]


Where can I get directions on how to complete the application?

Directions for completing the application are included on the form.


Do I need to submit a separate application form for each document I am requesting?

Yes. A separate application form [PDF] and processing fee [PDF] must be submitted each time an individual applies for a new document or renews an existing or expired document.
