How do I apply for the credential?
California Trained
You must apply for your credential through the California college or university where your program was completed.
Out-of-State Trained
First-time applications for credentials must include all of the following:
- a completed application [PDF]
- any supporting documentation (i.e., transcripts, copies of out-of-state credentials, etc)
- application processing fee [PDF]
- a copy of the Request for LiveScan Service Form [PDF] must be attached to the application packet as proof that fingerprints were taken and the fingerprint processing fees paid. Out-of-state residents only (with out-of-state addresses) have the option of submitting two completed fingerprint cards (FD-258) in lieu of a LiveScan receipt. If submitting fingerprint cards, fingerprint processing fees must accompany the application packet in addition to the application processing fee. Fingerprint cards may be ordered by sending an e-mail to CAWOrders@ctc.ca.gov. This e-mail address is for ordering materials only. Credential questions sent to this e-mail address will not be answered.
