I never received my document. What do I do now?

If this page shows that your document has been granted and mailed to you, submit a completed Duplicate/Replacement Request Form [PDF] to the Commission office at the address noted on the form. If the replacement request is made within one year from the date the document was mailed by the Commission, the replacement will be issued free of charge. If the replacement request is made after that year is up, an application processing fee of $27.50 is required. Personal checks, cashier's checks, and money orders are all acceptable forms of payment and should be made payable to the California Commission on Teacher Credentialing or CCTC. Mail your packet to the address noted on the top of the form.
If this page shows that your document is still pending evaluation, please know that applications are processed in the order in which they are received. Staff cannot search for pending applications or process applications out of order. If you are employed, please contact your employing agency concerning issues related to pay and issuance of your credential.
