How can I obtain a duplicate copy of my document?
Submit a completed Duplicate/Replacement Request Form [PDF] and an application processing fee of $27.50 . Personal checks, cashier's checks, and money orders are all acceptable forms of payment and should be made payable to the California Commission on Teacher Credentialing or CCTC. Mail your form and processing fee to the address noted on the top of the form.
