Adding Authorizations to an Existing Credential
How to Apply
To add an authorization to an existing credential, you must submit the following:
- A completed application (form 41-4) [PDF]
- A complete set of official transcripts showing the course work that are required for the authorization you are requesting. The Commission will accept photocopies of transcripts only if the application is filed through an employing agency.

Note: If you are applying for an added authorization (elementary to secondary or vice versa) at the same time you apply for your initial credential, a separate application and fee is required for each.
If you are adding an additional subject to a Single Subject Teaching Credential, you may submit one application at the time you renew your Single Subject Teaching Credential.
