Educator Discipline
The Division of Professional Practices investigates allegations of misconduct by credential holders and applicants. The Committee of Credentials reviews the allegations and may recommend to the Commission adverse action against a credential or application. The Commission votes whether to accept the recommendations of the Committee.

File a Complaint Against an Educator [PDF]
Use this form to file a complaint of misconduct against an educator. To file a complaint, you must have first hand knowledge of misconduct. However, the quality of a credential holder's service is most often an employment issue and not misconduct. Contact the educator's employer if you have concerns. -
Applicants should use this form to explain any criminal convictions. [MS Word]
When applicants apply for a credential, they must include an explanation of every conviction and other misconduct. The explanation must be included with their application. In addition to the explanation, applicants must send certified court documents showing plea and sentencing and the police report.
